Apollo Payroll Deducted Limited Group Health Insurance Plan
SRC - An Aetna Company is the provider for the Apollo Contract worker group health plan. This plan is a "Limited Accident and Sickness PPO Insurance Plan" and is available throughout the country via the Aetna Network. The plan offers limited benefit coverage for individual, individual plus one and family. It is considered limited because is contains caps on the benefits it pays. Please consult the attached 2010 Aetna Enrollment Kit for more details.Be advised that since the premium is a pre-tax weekly payroll deduction, changes are only permitted to your plan during the open enrollment period, which falls on January 1st of each year.
Enrollment in the insurance plan is available to any Apollo employee upon start of employment and goes into effect the first of the following month.
